PROGRAM: Game Time Sports Service L.L.C

ACTIVITY: Basketball Training / After School Activities / Youth Leagues / Sports

Please read this Acknowledgement of Risk and Waiver of Liability for Youth Program carefully and in its entirety; it is a binding legal document.

This agreement is entered into by the undersigned parent or legal guardian of the minor child (“Participant”) in favor of Game Time Sports L.L.C., its directors, officers, employees, coaches, agents, affiliates, volunteers, and facility partners (collectively referred to as “GTS”).

1. Acknowledgement of Risk

I understand and acknowledge that participation in youth basketball, including training, games, after-school activities, travel, conditioning, and all associated programming, involves inherent and unforeseen risks of injury, including but not limited to bruises, sprains, concussions, fractures, permanent disability, or death.

2. Medical Consent

I certify that my child is physically fit to participate and I have adequate health insurance to cover any medical treatment required. I authorize Game Time Sports L.L.C. to seek emergency medical care if needed.

3. Waiver and Release of Liability

In full consideration of the acceptance of my child’s participation in GTS activities, I, for myself and on behalf of my child, hereby voluntarily and knowingly:

• WAIVE any and all claims, causes of action, or liabilities against Game Time Sports L.L.C., its staff, and facility providers arising from or related to any injury, accident, illness, property damage, or death occurring during or related to participation;
• RELEASE GTS from liability, whether caused by negligence, carelessness, accident, or otherwise, to the fullest extent permitted by UAE law;
• ACKNOWLEDGE that this release covers all GTS programming, including but not limited to practices, games, scrimmages, off-site events, and transportation to and from such events.

4. Indemnity Clause

I agree to indemnify and hold harmless Game Time Sports L.L.C. from any claims or expenses resulting from my child’s actions or participation in the program.

5. Code of Conduct

I agree to support and encourage respectful behavior from both myself and my child. I understand that failure to follow program rules may result in removal from activities.

6. Legal Compliance

This waiver is governed by the laws of the United Arab Emirates. If any part of this waiver is deemed unenforceable, the remaining provisions remain valid.

7. Media Release

I acknowledge that Game Time Sports L.L.C. (the "Program") may capture video recordings, photographs, and audio recordings of my child while participating in basketball training sessions, games, after-school activities, tournaments, events, and other related sports programming (“Activity”).

I authorize the Program to record, use, and publish my child’s image, likeness, name, voice, and general biographical information in any media format—whether now known or developed in the future—including, but not limited to:

• Promotional videos
• Social media posts
• Website content
• Training highlights
• Marketing and advertising materials
• Printed publications
• Sponsorship decks and community outreach presentations

I understand and agree that these materials may be used without geographic or time limitation, and without further notification, review, or compensation.

If I choose to opt out, I will notify Game Time Sports L.L.C. in writing before my child begins participation. The absence of such notification shall serve as confirmation of consent.
