Acknowledgement of Risk, Waiver of Liability & Consent

PROGRAM: Game Time Sports Services L.L.C (GTS)

ACTIVITY: Basketball Training / After-School Activities / Youth Leagues / Sports Programs

By registering through the GTS app, the parent or legal guardian (or participant if over 18) agrees to the following terms and conditions.

^{1. Acknowledgement of Risk}

I understand and acknowledge that participation in basketball and related sports activities involves inherent risks, including physical exertion, contact with other participants, use of sports equipment, slips, falls, and other accidents that may result in injury.

I acknowledge that injuries may include, but are not limited to, bruises, sprains, strains, fractures, concussions, serious injury, or in rare cases, permanent disability or death.

I voluntarily assume all known and unknown risks associated with participation in GTS programs.

^{2. Medical Fitness & Emergency Consent}

I confirm that the participant is physically fit to participate and that all relevant medical conditions, allergies, or limitations have been disclosed during registration.

I confirm that I maintain adequate medical insurance coverage.

I authorize Game Time Sports Services L.L.C (GTS) to seek emergency medical treatment for the participant if deemed necessary and understand that I am financially responsible for any associated medical costs.

^{3. Waiver and Release of Liability}

To the fullest extent permitted under the laws of the United Arab Emirates, I hereby waive, release, and hold harmlessGame Time Sports Services L.L.C (GTS), its owners, employees, coaches, contractors, volunteers, and approved facility partners from any claims, demands, or liabilities arising from participation in GTS programs, except in cases of gross negligence or willful misconduct.

This waiver applies to all GTS activities, including training sessions, games, camps, leagues, and sanctioned off-site events.

^{4. Code of Conduct}

I agree that the participant and parent/guardian will conduct themselves respectfully toward coaches, staff, officials, and other participants.

Failure to comply with GTS rules, policies, or behavioral standards may result in suspension or removal from activities, without refund.

^{5. Media Consent}

I acknowledge that photographs or video recordings may be taken during GTS activities.

I consent to the use of the participant’s image or likeness for GTS promotional, educational, and marketing purposes, unless I notify GTS in writing prior to participation.

^{6. Governing Law}

These Terms & Conditions shall be governed by and interpreted in accordance with the laws of the United Arab Emirates.

By completing registration and/or payment through the GTS app, I confirm that I have read, understood, and agreed to these Terms & Conditions.
